Questionnaire

The LP is designed to generate data to accurately forecast the filtration media life-cycle.

1/ You must download the THERMO LP Survey form and complete it.
2/ Then, create an account or connect to the LP Report Area with the information you will receive after your account has been created.
3/ Upload the THERMO LP Survey form, once completed, check your information and send it to our lab.

Your questionnaire can be modified while it is being processed by our sales technician.
Our lab technician will respond to you within 48 hours with an LP Report in accordance with your handling descriptions.

Calculated Payback

Based on the calculated life-cycle from step 1 we forecast the savings of using Neutrodine versus the energy cost associated with the classic approach of ducting to atmosphere.

Non Disclosure Statement

	All data collected by erlab for the purpose of the L.P. Life-cycle Pay Back Survey will remain confidential.
	Erlab & THERMO undertake not to disclose any data to third parties.
	Erlab & THERMO shall restrict disclosure of any data to its employees with a need to know.
